Information Practices Statement

County of Lambton Long-Term Care Homes

The County of Lambton’s Long-Term Care Division provides a broad range of health services to residents in Lambton Meadowview Villa, Marshall Gowland Manor and North Lambton Lodge and to clients of the Adult Enrichment Centres. To meet residents’ and clients’ needs and provide effective care and service, Administration needs to collect, use and disclose personal health information. As such, Administration is the health information custodian, as defined in the Personal Health Information Protection Act 2004 (PHIPA) and is accountable for the protection of residents’ and clients’ personal health information.

Notice of Collection under Freedom of Information and Protection of Privacy Act (FIPPA)

Under section 177 of the Long-Term Care Homes Act, 2007 (the Act), the Ministry of Health and Long-Term Care collects personal information about persons who work or provide services at a long-term care home. The ministry collects and uses this information for purposes related to the administration of the Act, including ensuring compliance with the requirements under the Act. The ministry may also disclose this information for these purposes.
